(Year 2-12) There are several forms available that address learning styles and classroom practices. These can be easily modified to suit your purpose.

(Year 3-10) These three surveys provide self-assessment of students’ interest in and enjoyment of each subject.

(Year 3-10) A self-assessment of students’ interests in and enjoyment of Physical Education, their assessment of its value and of their ability to handle the activities they are given.

(Year 8-12) A 40 item self-assessment inventory which asks students about such aspects as the enjoyment of mathematics, their ability to handle the work and their assessment of the importance of mathematics.

(Year 3-7) A 24 item self-assessment inventory which asks students about such aspects as the enjoyment of mathematics, their ability to handle the work and their assessment of the importance of mathematics.

(Year 6-12) 40 item inventories asking students about their interest in, enjoyment of, ability to handle, and the perceived value of science.

(Year 3-7) A self-assessment inventory that asks students about such aspects as the enjoyment of reading, books available at home, use of the local library, ability to handle set reading tasks.

(Year 3-10) Students’ opinions regarding their use and enjoyment of the library, and their assessment of their ability to use it to find resources for study and pleasure.
